Aww, that's nothing. Happens all the time on the Ikea Line, often with the results of one express rear-ending another at the station. Usually the 5-Fingered Hand of God, and his Almighty Screwdriver swoop down on the scene, and passengers aren't put out for more than about 45 seconds.

 

Man, I'd hate to be the person responsible for inspecting that turnout and determining the cause of the accident, knowing that every second spent making a determination was costing the company money and goodwill, and that a bad call could result in further catastrophe.

The split switch be caused by several things.  It could be the switch itself, a wheel flange issue or operator error. Most electric streetcar switches are controlled by turning the power off or leaving it on depending on the direction you want to go.  Turn the power off at the wrong point and you could throw the switch under the rear truck.  Or it could be a wireless switch like Toronto has which has memory and can surprise unsuspecting operators.
