Unfortunately Pakistan & India have terribly unsafe train systems. There are frequent accidents.

 

I remember these subburb trains full spead ahead with all doors open and people trying not to fall. There are drama quite every day in Mumbai, Bangalore, Chennai, Delhi etc ...
