Need (Japanese language) help making a reservation for a tour of the Pocky factory.

I want to make a reservation to tour the pocky reservation. There is a form online but I can't submit it without a Kanji name, and Japanese phone number.

I phoned them, and in my mininmal Japanese asked if anyone there spoke English. They said no, and that was the end of it.
